The type of personal information we collect - 

We may collect personal information from you when:

-You contact us through the website.

-You make a referral

-You register for our mailing list.

Personal information such as:

- Names of parents, children and referrers

- Contact details of parents, children and referrers

- Children’s date of birth

- Your bank details

- Personal characteristic such as

- Children’s ethnicity

- Language

- The reasons that a child has been referred for play therapy

- Attendance information such as sessions attended, number of absences and absence reasons

- Information from school, eg name of Headteacher/SENCO, child’s class, name of class teacher, any additional support 

- Relevant medical information.

- Special educational needs information. 

- Behavioral information.  

- Details of any relevant diagnosis (eg ASD, ADHD), health condition or disability

- Details of any other agencies involved with the child 

- Brief, factual notes on each therapy session

- Other details about you or your child that you or others provide to us

How we get the personal information and why we have it

We will only request and process data that is necessary for the purpose of therapy and that helps us to understand the child and meet their needs.

Most of the personal information we process is provided to us directly by the parent/carer for one of the following reasons:

 - to support child’s therapy

- to monitor and report on child’s progress

- to assess the quality of our services

- to comply with the law regarding data sharing

- process any payments

- fundraising

We also receive personal information indirectly, from the following sources in the following scenarios:

- Information received from the referrer in the referral form.

- Information received from school when this is to support the child’s therapy

- Information received from other agencies to support the child’s therapy.

We do not share information about our clients with anyone without consent unless the law and our policies allow us to do so. For example if a child discloses abuse or we believe a child to be at risk.

 

We may share this information with: 

​

- People you have agreed for us to share it with

- People you have authorised to act on your behalf

- Emergency services or other agencies where there appears to be a serious risk of harm to you, your child or someone else

- Judiciary services if I am instructed to do so by the courts

- Where companies are providing services on our behalf, such as processing donations or orders.

- You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.

How we store your personal information 

Your information is securely stored in a locked cabinet. By law, we need to keep records on children until they are 26 years old.  They will then be disposed by being shredded.
